HAVE YOU EVER WORKED AT MALLINCKRODT CHEMICAL COMPANY OR ANY OF THE LISTED 350 FACILITIES AS A WORKER, FORMER WORKER OR SUBCONTRACTOR? (THIS INCLUDES CONSTRUCTION, DISMANTLING, CLEANUP, ETC.)
ARE YOU A SURVIVOR OF A FORMER WORKER OR SUBCONTRACTOR THAT HAS PASSED AWAY?
The Energy Employees Occupational Illness Compensation Program Act became effective July 31, 2001 and provides for a lump-sum payment of $150,000.00 and payment of medical expenses to workers, former workers and subcontractors who became ill as a result of their exposure to radiation, beryllium, or silica. Conditions covered under this program are: radiogenic cancers, beryllium diseases and chronic silicosis. Qualified survivors of a covered worker are also eligible for compensation, i.e.: spouse, children, grandchildren. Another program, (Subtitle D), is a state workers compensation that can be filed for those who worked at certain facilities and have a (covered illness) non cancerous, but caused by or aggravated by radiation exposure, i.e.: pulmonary problems, blood disorder, muscle and bone deterioration, etc.

Weekly Steward Reports must be turned in weekly to the Financial Secretary. The workweek begins on Wednesday and ends on Tuesday.
Important: Fill in hours worked. When reporting an injury, please be sure to include injury date and specific injury.
